New Patient Registration
The doctors welcome new patients who live within our practice area. Please bring in your medical card, photo ID (passport or driving licence) and proof of address. Children under 16 do not need proof of ID but they must have a parent or guardian registered at this practice. If you do not possess a medical card please ask at reception for an application form. Temporary residents are registered on a special form provided at reception. After registration you can make an appointment to see any doctor (regardless of who your registered doctor is).
New patients will be required to complete a simple personal health questionnaire as it can take time for records to be forwarded from your former practice. We also require you to provide a blood pressure reading which can be obtained using one of the two machines in the surgery. You are invited to make an appointment with the health care assistant for a basic health screen check. This primarily consists of a blood pressure and urine check. Your height and weight will be measured and details of your previous medical history will be recorded onto your medical records. Please bring a urine sample with you in a clean container.
