How to register

If you live within our practice area (click here for a boundary map) you can register by printing off and completing our Registration Form and questionnaire.
You will then need to bring them to the surgery along with proof of ID and Residency:
EITHER
- UK photo card Driving Licence showing your current address
OR
- Current passport, AND
- Bank statement or utility bill (dated within the last 3 months) OR Council Tax bill (current year)
You may want to avoid the busiest times at reception and visit the surgery between 11am and 2pm so we can process your application as smoothly as possible. During registration we will also ask you to provide a blood pressure reading using our machine in the reception area.
Once registered, you can make an appointment to see any doctor regardless of who your registered doctor is. Your records will automatically be transferred from your previous surgery. This can take a few weeks so it is important to complete our questionnaire as fully as possible.
Children under 16 years
Children under 16 do not need proof of ID but they must have a parent or guardian registered at this practice. Please use this questionnaire (under 16) to register a patient under the age of 16. The same registration form as above can be used for patients under 16 years.
